If you are 15 years old or older, you will need to apply for an adult VA ID card. You will need: A completed Driver's License and Identification Card Application (Form DL 1P).
In the present time, a driver’s license and an identification card are some of the items people would consider as a necessity. These cards serve as proof of identity, which is needed in availing of government services, in making business transactions, in applying for jobs, and many more.
Aside from being a proof of identity, a driver’s license is important as it ensures that you value safety when traveling. If you’re not yet eligible for a driver’s license, you can choose to get a learner’s permit--it almost does the same job as a driver’s license.
Whether you need a driver’s license, learner’s permit, or an ID card, you can apply for one at the DMV by filling out Form DL 1P.
Form DL 1P is filled out when applying for a driver’s license, learner’s permit, or ID card.
If your DL or ID card is lost, stolen, or destroyed, you will also have to submit this form.
If you have undergone surgery to improve your vision restriction, then you are also required to submit an accomplished Form DL 1P to remove the vision restriction indicator in your driver’s license.
ID Card
Anyone who is a citizen of Virginia can avail of an ID card. If you are at least age 15, you may apply using Form DL 1P. If you are under age 15, applying for an ID card, you would have to fill out a different form--Form DL 5
Driver’s License
You must be at least 16 years and 3 months of age and a citizen of Virginia. If this is your first time applying for a driver’s license, or if you cannot show proof that you previously held a license, the DMV requires you to show proof of either of the following:
a. You have passed a state-approved driver education course
b. You have held a learner’s permit at least 60 days before taking the DMV driving test.
Learner’s Permit
You must be at least 15 years and 6 months of age and a citizen of Virginia. You must also complete a state-approved driver education program before applying for a learner’s permit.
Before filling out the form, make sure to read through the form first to make sure that you give out all the necessary information.
Voter Registration and Organ Donation
On the topmost part of the form, you will see a section for voter registration and organ donation. The DMV highly encourages you to fill out this section, but it is not required for qpplying a driver’s license, learner’s permit, or an ID card.

In this section of the form, you will be asked about the type of card you will be applying for. Check the box next to your answer.
Applicant Information
Input the following in the spaces provided.
In the lower part of the section, you will be asked additional questions, and you will also be given a choice to request for a special indicator in your ID card. Read the items in these sections carefully and answer accordingly.
On the continuation part of this section, you will also be asked if you currently hace a license, ID card number, or learner’s permit from another state, US territory, or foreign country. Please answer the items that apply to you.
Parent or Legal Guardian Consent
This section is intended to be filled out by minors applying for an identification card or a learner’s permit (at least 15 years and 6 months of age), and for minors under age 15 applying for an ID card.
The parent should give his or her full name and signature.
Selective Service
Male US citizens aged at least 18 and younger than 26 years old are mandated by law to take part in the Selective Service Program. Veteran Indicator
If you are a veteran, you may choose to add a veteran indicator in your driver’s license or ID card.
Government Employees
If you are a government employee, let the DMV know through this form. Government employees are entitled to a waiver of the motorcycle class endorsement fee.
