To replace a missing, damaged, or destroyed title, you'll need to complete the Utah Duplicate Title Application Form, Form TC-123. An owner of the vehicle must sign the title. If your mailing address differs from the street address that is associated with your title, be sure to fill out section 1 of the application.
Form TC-123, Application For Utah Duplicate Title, or also known as Utah Duplicate Title Application Form is a document by the Utah State Tax Commission that vehicle owners in the state must fill out and submit to apply for a duplicate certificate of title.
It is used if the original certificate was damaged or lost. A seller of a vehicle should use this when selling to a Utah resident. In this case, the owner must fill out the Utah Duplicate Title Application Form and give it to the purchaser. A week after receiving the application, the Utah Division of Motor Vehicles provides a duplicate.
