Enter the names of any current co-tenants on the property.
Property
Enter the name of the property.
Term of Agreement
Enter the term of your lease agreement (the starting date and the end date).
This section further declares that the New Tenant understands that they are entering a binding legal agreement that can and will be enforced by the Landlord or Principal Tenant and other roommates, and that if the owner of the property is not also living on the property, that this agreement will not protect them or any of their roommates against any claims that the property owner may have against them.
Section 2 - Security Deposit
Deposit Amount
Enter the amount of the provided security deposit for the premises. Then enter the total of all deposits provided by all roommates and the provided security deposit.
Liability
The New Tenant accepts that they will be held responsible for any damages that they, their pets, or their guests cause, and they will reimburse the other roommates for the part of the security deposit withheld for the damages.
Section 3 - Rent
Rent
Enter the total amount of rent that must be paid per month, then the amount that the New Tenant agrees to pay per month upon signing this agreement.
Payment Receiver
Enter the name of the person or entity to whom all payments must be made.
Eviction
This section declares that the New Tenant understands that the owner of the property can evict all of the tenants if the Landlord does not receive the rental payments in full and on time each month.
Section 4 - Utilities
Utility Payment Allocation
Enter the fraction of the payment for utilities and services that the New Tenant will pay, and for which utilities and services.
Utility Responsibility
Enter the utilities and services that the New Tenant agrees to take responsibility for and ensure that monthly payments are collected and made by all co-tenants in full and on time for the maintenance of said utilities and services.
Additional Utilities
Enter any additional utilities and services that the New Tenant agrees to pay for.
Section 5 - Move-Out
This section declares that the New Tenant must take on the responsibility of finding a replacement tenant if they decide to move out of the rental premises before the lease is concluded. Additionally, should any other roommate move out, the primary responsibility to find a replacement tenant will fall to the Landlord or Principal Tenant.
Section 6 - Additional Agreements
Attach a description (written on a separate sheet of paper) of any and all additional agreements that will be added to this form. These may include, but are not limited to, the following:

Enter the state under whose laws the lease will be governed by.
Section 8 - Authorization
Enter the day, month, and year that the agreement was executed.
Principal Roommate/Landlord’s Signature
Have the Principal Roommate or Landlord enter their name and sign the form in the spaces provided.
New Roommate’s Signature
Have the New Tenant enter their name and sign the form in the spaces provided.
Co-Roommate Signatures
Have each of the current co-roommates enter their name and sign the form in the spaces provided.
