What is the Pennsylvania Change of Address form?
The Pennsylvania Change of Address form is a document used when you are moving to a new address within the State of Pennsylvania.
This allows the government or state officials to update your records. When you change your address, it will also be reflected on your driver’s license, vehicle registration, and other identification that contains your address.
How to fill out the Pennsylvania change of address form?
NOTES
- The State of Pennsylvania requires you to update and report your new address to the Department of Transportation within 15 days of moving.
- If you identify a mistake on your address you must notify the Department of Motor Vehicles (DMV) or state officials regarding the mistake and you must change your address.
- When you change your address for a vehicle registration, the DMV will also update the address on the title certificate.
- It is recommended that you order new DMV documents when you change your address, but it is not required. If you do not order new documents, you may write your new address on the back of your driver’s license, non-driver ID card, or on the front of your registration document. Do not write your new address on a title certificate.
- Documents and important notices are sent by the DMV by mail, it is recommended that you use the same address on all of your applications, documents, and transactions.
- The United States Postal Service (USPS) does not notify the DMV when you change your mailing address. If you change your address with the USPS, they do not update your DMV records, therefore, you must report your address change to both the USPS and the DMV.
- When you update your address for a Traffic Violations Bureau (TVB) ticket transaction, the update does not change the address on your driver’s license, non-driver ID, permit, or vehicle registration records.
- When you update your address for your driver’s license, non-driver ID, permit, or vehicle registration records, it does not automatically change your address for your traffic violation ticket.
- The information that you provide on this form is strictly for official purposes only. It will only be used by appropriate offices and officials. Your information is under the Data Privacy Act and shall not be released to the public without your consent.
- The information that you provide must be true and free of any misleading information. If you enter false information, it may result in complications and you may be fined or imprisoned.
INSTRUCTIONS
- Type and print legibly and carefully in ink to avoid further issues or complications.
- The information that you enter must be complete and accurate. When you change your address on your vehicle registration only, include your driver’s license or photo identification number in Section A.
- Be informed that this form is not valid for name changes or commercial driver’s license (CDL) updates. If you are currently holding a CDL, use form DL-80CD, and indicate the appropriate fees.
- An address in the State of Pennsylvania can only be recorded on title and registration products. All of your change of address applications must indicate your actual or bona fide Pennsylvania numbered street address.

These are the only exceptions to motor vehicle documents being processed with an out-of-state address.
- United States Armed Forces personnel or their dependents
- An employee of federal or state government or their family members whose workplace is located outside the State of Pennsylvania.
- A business that is located outside the State of Pennsylvania and has no Pennsylvania location and the vehicle is regularly used for business operations within this commonwealth; or,
- A Park Model Trailer permanently located in Pennsylvania.
If you are entitled to one of these exceptions, you must complete Form MV-8 or the Self Certification for Proof of Residency form and attach it to this form.
- Mobile home vehicle records are exempt from the Pennsylvania address requirements and are not required to submit a Form MV-8 when titling the vehicle to an out-of-state address.
- Your county of residence may impose an annual fee of $5 for each motor vehicle registered in that county. Pennsylvania Department of Transportation will collect the $5 fee at the time when a vehicle is registered or when a registration is renewed. The funds collected will be used by the county for transportation purposes or be allocated by the county in accordance with Section 9010(c) of the Pennsylvania Vehicle Code.
A. Driver’s License or Photo ID
You may enter two (2) customer names at one address.

Date of Birth
Enter your date of birth.
Customer Name
Enter your full name using the following format: Last, First, Middle name.
B. Vehicle Registration
You may enter up to three (3) vehicles at one address. If the vehicle(s) is or are leased, contact your leasing company to have Form MV-1L submitted for vehicle registration address change.
Title number including alpha characters
Enter the title number of the vehicle and include the alpha characters.
Vehicle make
Identify the make of the vehicle.
Registration Plate Number
Provide the registration plate number of the vehicle.
C. New Address
Street
Enter the street name of your new address. You can use a P.O. box number in addition to the actual street address but it cannot be used as the only address.
City
Enter the city of your new address.
State
Enter the State of your new address.
Zip code
Enter the Zip code of your new address.
D. Voter Registration Update
Mark YES if you are a registered voter in the State of Pennsylvania and you would like to notify your county voter registration office of your address change. Mark NO if otherwise.
E. Person with disability parking placard update
If you have a person with disability parking placard and require an address change, you must complete this section.
