Residents of New York can use it to renew their learner’s permit, driver’s license, or non-driver ID in New York. Aside from its use as a renewal form, residents can also use it for the following for other purposes, such as applying for a new or replacement driver’s license, getting a conditional or restricted driver’s license, changing information in an existing driver’s license, and transferring out-of-state driver’s license to New York.

Existing Driver License
Mark “Yes” if you have ever had a driver license that is valid or that expired within the last two years, issued by another US State, the District of Columbia, or a Canadian Province. Date of Birth
Enter your date of birth in the following format: Month, Day, Year.
Gender
Mark “Male” if you’re male. Check “Female” if you’re female.
Height
Enter your height in feet and inches.
Eye Color
Enter your natural eye color.
Telephone Number
Enter your telephone number, including its area code.
Name Change
Mark “Yes” if your name has changed. Then, enter your former name as it appears on your present license or non-driver ID card. Otherwise, mark “No.”
Other Change
Enter the change and the reason for the change.
Social Security Number (SSN)
Enter your Social Security Number. Otherwise, mark the appropriate box if you have never been issued a Social Security Number.
Address Where You Get Your Mail
Enter the address where you receive your mail. Include the street number, street name, apartment number, city or town, state, zip code, and country.
Address Where You Live
If your residential address is different than your mailing address, provide your residential address. Include the street number, street name, apartment number, city or town, state, zip code, and country.
Has Your Mailing Address Changed?
Mark “Yes” if your mailing address has changed. Otherwise, mark “No.”
Has the Address Where You Live Changed?
Mark “Yes” if your mailing address has changed. Otherwise, mark “No.”
If you answered “Yes” to either of the questions above, then addresses on all vehicle registrations tied to your ID number will also be updated with the address you provided, unless you check the appropriate box.
If you are registered to vote, your registration record will be updated when you complete and submit this form. Mark the appropriate box if you do not want your new address on your voter registration record. If you do not check the box, your new address will be sent to the Board of Elections of your county of residence.
Veteran Status
If you are a veteran and you would like to have “Veteran” printed on the front of your photo document, check the appropriate box. Present proof of your honorable discharge from military service.
New York State Organ and Tissue Donation
If you would like to be added to the New York State Donate Life Registry as an organ donor, mark “Yes” and sign and date on the space provided. Otherwise, check “Skip This Question.”
If you would like to donate $1 to the Life...Pass It On Trust Fund for organ and tissue donation research and outreach, mark the appropriate box.

Voter Registration Questions
If you are not registered to vote where you live now and would like to apply to register, mark “YES” and if you’re not submitting this form to a DMV office, complete the voter registration application section. Mark “NO” if you decline to register or if you are already registered.
If you do not check any of the two boxes, the DMV will consider that you have decided not to register to vote.
Registrations with the US Selective Service System (SSS)
If you wish to not register for the SSS and not get its benefits, mark “NO.”
General Questions
1. If your driver license, learner permit, or privilege to drive a motor vehicle been suspended, revoked, or canceled or has your license application been denied in this state or elsewhere, mark “Yes.” Otherwise, mark “No.” If you answered “Yes” to the preceding question answer the following question:
a. Has your license, permit, or privilege been restored, or has your application been approved? If yes, mark “Yes” and if no, mark “No.”
2. If you have received treatment, currently receive treatment, or take any medication for any condition that causes unconsciousness or unawareness, mark “Yes.” Otherwise, mark “No.”
3. If you need a hearing aid and/or a full view mirror to drive a motor vehicle, mark “Yes.” Otherwise, mark “No.”
4. If you have lost the use of a leg, arm, hand, or eye, mark “Yes.” Otherwise, mark “No.”
a. If you need to renew your driver license, marked “Yes” on the previous question, and the injury occurred since your last driver license, mark “Yes.” Otherwise, mark “No.”
b. If you marked “No” on the previous question and your condition has gotten worse since your last driver license, mark “Yes.” Otherwise, mark “No.”
Parent/Guardian Consent
Mark the appropriate box regarding whether the form is for a junior license or a non-driver ID card.
If you are applying for either, provide the signature and relationship to the applicant of a parent or guardian as well as the date of signing the space provided.
Teen Electronic Event Notification Service (TEENS)
If you would like to enroll in the TEENS program, provide the ID number on a New Tork State driver license, permit, or non-driver ID card of the consenting parent or guardian that was provided above.
Commercial Driver License Applicants Only
1. If in the past 10 years a driver license was issued to you from another state in the US or the District of Columbia, mark “Yes” and provide the name of each state. Otherwise, mark “No.”
2. Mark the appropriate box you wish to certify to DMV the commercial motor vehicle that you operate or expect to operate:
