Form MV-44, Application for Permit, Driver License or Non-Driver ID Card, is a form by the New York Department of Motor Vehicles (DMV) that New York residents use to apply for or renew a driver’s license, learner’s permit, or non-driver ID in New York.
Form MV-44 requires your personal information and requires you to answer questions relevant to being a capable driver. This how-to section will give you step-by-step instructions on how to fill out the items in the form correctly.

Application For
Depending on what you are applying for, you will mark the appropriate checkbox. “License,” “Permit,” and “ID card” are the options. Since you will apply for a driver’s license, mark the “License” checkbox.
Purpose For Application
Depending on the purpose of your application, you will mark the appropriate checkbox. “New,” “Renew,” Update Info,” Change Type,” “Replacement,” “Conditional,” “Restricted,” and “Transfer to NY” are the options.
Identification Information
Mark the “Yes” checkbox if you now have or ever had a New York driver’s license, learner permit, or non-driver ID card; otherwise, mark “No.” Then, provide the ID number on NYS Driver License, Learner Permit, or Non-Driver ID Card.
Note that applying for a Non-Driver card will cancel any NY State driver license privilege.
Full Last Name
Enter your last name.
Full First Name
Enter your first name or names.
Full Middle Name
Enter your full middle name.
Suffix
Enter your suffix.
On the right side of the names section, mark the “Yes” checkbox if you have or ever had a driver license that is valid or expired within the last two years, issued by another U.S. state, the District of Columbia, or a Canadian Province; otherwise, mark “No.” If you answered yes, provide Where It Was Issued, the Date of Expiration, Type of License, and Out-of-State License ID Number.
Date of Birth
Enter your date of birth in the following format: Month, Day, Year.
Sex
Select your gender. Mark the “Male” checkbox if you are male and the “Female” checkbox if you are female.
Height
Enter your height in Feet and Inches.
Eye Color
Enter your natural eye color.
Telephone Number (Home/Mobile)
Enter your telephone number, including the Area Code.
Has your name changed?
If your name has changed, mark the “Yes” checkbox then, enter your former name exactly as it appears on your present license or non-driver ID card. Otherwise, mark “No.”
Other Change
If there is another change, enter it in the space provided and the reason for it. It may be a new license class, a wrong date of birth, or another valid reason.
Social Security Number
Enter your unique nine-digit Social Security Number.
Address Where You Get Your Mail
Enter the full address where you get your mail. Include Street Number and Name, Rural Delivery, and/or box number. If P.O. box, you must also answer the “Address Where You Live” item. Note: this address will appear on your standard identity document.
Address Where You Live
Enter the full address where you live. This item is required if different from the address where you get your mail. Do not enter a P.O. box. Note: this address will appear on your enhanced/real ID identity document.
Has Your Mailing Address Changed?
Mark the “Yes” checkbox if your mailing address has changed; otherwise, mark “No.”
Has the Address Where You Lived Changed?
Mark the “Yes” checkbox if the address where you lived changed; otherwise, mark “No.”
If you answered yes to either of the last two questions above, then addresses on all vehicle registrations tied to your ID number will also be updated with this address, unless you mark the appropriate checkbox under the “Has Your Mailing Address Changed” and “Has the Address Where You Lived Changed” items.
If you are registered to vote, your voter registration record will be updated when you complete and submit Form MV-44. If you do not want your new address on your voter registration record, mark the appropriate checkbox under the “Has Your Mailing Address Changed” and “Has the Address Where You Lived Changed” items. If you do not check the box, your new address will be sent to the Board of Elections of your country of residence.
Veteran Status
Mark the checkbox if you would like to have “Veteran” printed on the front of your document. If you mark the checkbox, you must present proof that indicated an honorable discharge from military service.
New York State Organ and Tissue Organization
If you would like to enroll in the New York State Donate Life Registry, mark the “Yes” checkbox and provide your Signature and the date when you signed. You may also mark the “Skip This Question.”
In addition, if you want to make a $1 voluntary donation to the Life...Pass It OIn Trust Fund for organ and tissue donation research and outreach, you may mark the appropriate checkbox.
Voter Registration Questions
If you are not registered to vote where you live now and would like to apply to register, mark the “YES — Complete Voter Registration Application Section” checkbox; otherwise, mark the “NO — I Decline to Register/Already Registered.”
The following questions must be completed for ALL license /permit transactions.
Mark the “Yes” checkbox if applicable; otherwise, mark “No.”
4a. If you need to renew your driver’s license and you marked “Yes,” did this occur since your last driver’s license.
4b. If you marked “No” to 4a, has your condition gotten worse since your last driver’s license?
Parent/Guardian Consent
Mark the appropriate checkbox regarding whether the form is for a Junior License or a Non-driver ID Card (under 16).
If you are applying for either, provide your Signature, Relationship to the Applicant, and the Date of signing.
Teen Electronic Event Notification Service (TEENS)
If you would like to enroll in the TEENS program, enter ID Number on NYS Driver’s License, Permit, or Non-Driver ID Card of Consenting Parent or Guardian.
Commercial Driver License Applicants Only
Mark the “Yes” checkbox if in the past 10 years, a driver’s license was issued to you from another state in the U.S. or the District of Columbia; otherwise, mark “No.”
