This form is used to request a change of address for your regular or enhanced driver license, non-driver photo ID card and vehicle registrations. You can also order a new document with your new address.
Form MV-232, Address Change, or the New York Change of Address form is a state-level government form issued by the New York Department of Motor Vehicles (DMV) that residents of New York can use to change the address on their regular or enhanced driver’s license, non-driver photo identification card, and vehicle registrations.
The DMV is a government agency responsible for vehicle-related services, including issuing licenses, registering vehicles, and, maintaining a database that contains the driver information of its residents.
The New York state of law requires New York residents who moved within the state or from another state, to notify the DMV within 10 days of any permanent address change. There is no need to report a temporary change of address.
You may or may not apply for a new license, permit, non-driver identification card, or registration document when you change your address with the DMV. Getting new documents is available at additional costs.
When you change your address, you can write the new one on the back of your driver’s license or identification card and your vehicle registration document, crossing off your old address.

The New York Change of Address form is a two-page form. The first page contains instructions and the second page contains the form with three parts. To apply for a new document, complete Part 1 and Part. Otherwise, if you do not want a new document, complete Part 1, but do not answer Part 2. Part 3 is for the voter registration application.

Enter the new address where you get your mail. Include the street number and name, rural delivery, apartment number, and/or P.O. box number. Also fill in the residence address including city or town, state, ZIP code, and county.
Mark the appropriate box if the address where you get mail is the same as the address where you live. If you mark the box, do not complete the next “New Address — Where You Live” item and answer the “Previous NYS Address” item.
New Address — Where You Live
Enter the new address where you love. Do not give P.O. box. Include the street number and name and apartment number. Also enter your city or town, state, ZIP code, and county.
Previous NYS Address
Enter your previous address in New York, including the street number and name, rural delivery, apartment number, and/or box number.
Registration
Enter the information required for each registration in your name that you would like updated to your new address. Enter the License Plate Number and mark the Type of Registration for each vehicle. For the Type of Registration, you may select:

Part 3 is used to register to vote or update your voter registration information. Only complete this part if you want to register to vote or update your voter registration information with the Board of Elections.

Mark Yes if you are a citizen of the U.S.; otherwise, mark No and you cannot register to vote.
Mark Yes if you will be 18 years of age or older on or before election day; otherwise, mark No and you cannot register to vote.
Mark Yes if you are at least 16 years of age and understand that you must be 18 years of age on or before election day to vote and that until you will be eighteen years of age at the time of such election your registration will be marked “pending” and you will be unable to cast a ballot in any election; otherwise, mark No and you cannot register to vote.
Mark Yes if you have voted before and give the year; otherwise, mark No.
