Use this form to notify the Motor Vehicle Administration of a change of residence or mailing address for your vehicle, vessel, driver license, or identification card records.
TheMaryland Change of Address form is issued by the Maryland Motor Vehicle Administration (MVA) and used by eligible applicants to notify the MVA of any change in the applicant’s address for their Maryland driver's license.
The applicant can also fill out this form to apply to register to vote in Maryland and to change their political party affiliation.
The Maryland Motor Vehicle Law requires that you notify the MVA of any change in your address within thirty days of the change.
Before accomplishing this form, make sure that the information you will provide is complete and accurate. Failure to provide complete and accurate information may result in delays in processing your application, denial of your application, and if the incorrect information is found intentional, penalties and repercussions, such as fines and imprisonment, may occur.
Provide the required information in print. Do not use cursive.
Provide all of the information required unless stated otherwise.
Provide the required information beginning with the leftmost region of the space provided and continue writing towards the rightmost region. If you have completed your answer and there is space left available, leave the space blank.
Before signing and dating the form, make certain that all of the information provided is complete and accurate.
Form VR-154 has two pages and four sections. The first page is for providing the required information for your application for Maryland change of address and voter registration. The second page is for information for your application for Maryland change of address and voter registration.
The four sections are section A, section B, section C, and section D.
Section A consists of the area where all applicant will provide their information regarding their Maryland change of address.
Section B consists of the area where those who want to apply to register to vote in Maryland or change their party affiliation will provide their voter registration information.
Section C consists of information that must be read by all applicants.
Section D consists of information that must be read by those who want to apply to register to vote in Maryland or change their party affiliation.
Section A. All Applicants
Line 1: Driver’s License or Soundex Number
Provide your driver license number. If you don’t have a driver license, provide your Soundex number instead.
Line 2: Date of Birth
Provide your date of birth.
Line 3: Name
Provide your first name, followed by your middle name, then your last name, and finally your title or suffix if you have any.
Line 4: Old Resident Address
Provide your previous residential address. Include the number, street, city, county, state, and zip code.
Line 5: New Resident Address
Provide your current residential address. Include the number, street, city, county, state, and zip code.
Line 6: Title Number
Provide the title number.
Line 7: Tag Number
Provide the tag number.
Line 8: Sticker Number
Provide the Sticker number.
Line 9: Signature
Provide your signature.
Line 10: Date
Provide the date you accomplished this form.
Section B. Voter Registration
If you are already registered to vote in Maryland and you only want to change your party affiliation, complete lines 3, 9, 13, and 14.
If you are not registered to vote in Maryland, a US citizen, and would like to apply to register to vote, complete lines 2 through 14.

Line 7: Daytime Phone Number
Provide your daytime phone number.
Line 8: Social Security Number
If you would like to provide your SSN, provide it. This part is optional and voluntary. If you choose to provide this information the number will only be used for registration and other administrative purposes. Your SSN will be kept confidential.
Line 9: Political Party
Check the political party you want to take part in that political party’s primary election, caucus, or convention. Check only one box.
