If you check the box and agree to become an organ and tissue donor, your consent will be clearly indicated on your identification card with a small red heart. Social Security Number disclosure statement
The Federal Privacy Act of 1974 is present in this form. This ensures that your social security number is disclosed to the public and can only be used to apply for or renew your driver’s license or non-driver identification card. It also states that your social security number will only be used for identification purposes and it will be kept confidential.
The last line of the application states that you should not provide false information on this form. If you do so, it is stipulated under Title 17-A that it is a Class D crime and is punishable by confinement up to 364 days or a monetary fine of up to $2000 or both.

If you hold a driver’s license that comes from any state or province, that license must also be presented. Do not present photocopies of the documents.
