The MCS-150, or Motor Carrier Identification Report, is a form filed to obtain and update your USDOT number.This allows the FMCSA to verify that the information associated with your DOT number is correct
The Federal Motor Carrier Safety Administration (FMCSA) is responsible for monitoring motor carriers and ensuring that they adhere to safety standards. These standards are set so that the property or the passengers aboard the carrier vehicles will be transported in good condition, and that motor-vehicle related fatalities and injuries will be prevented.
All motor carriers who are operating commercial motor vehicles are required to apply for a Department of Transportation (DOT number) before they can begin transporting passengers or properties. After registering for a DOT number, they must then file a Motor Carrier Identification Report to update their records.
The Form MCS-150 or their Motor Carrier Identification Report is one way the FMCSA could conduct the said monitoring. Companies are mandated to submit this report biennially (every two years) so that their records with the FMCSA are regularly updated.
From these forms, the FMCSA will be able to tally your company’s safety scores. Therefore, it is important that you provide the correct information so that the FMCSA could accurately determine your company’s safety scores.
The period when you should register is based on your company’s given USDOT number.

If you do not submit Form MCS-150 on time, then you will not be able to update your USDOT number. You will also be paying a fine of $1000 per day. Aside from that, your USDOT number may be deactivated, and it may affect the operations of your business.
Form MCS-150 is 11 pages long. The first eight pages contains the step-by-step instructions on filling out the form. The actual form, which you have to fill out, is three pages, starting from page 9. When you finish filling out the form, you only have to submit the last three pages.
This article only provides general instructions on filling out the form. As stated above, the first (5) pages of the form provide specific instructions on filling out each item in the form. Please read through these instructions carefully for clarifications, so that you avoid making mistakes while filling out the form.
1 . Legal Business Name: Input here the legal business name of your company.
2 . Doing business as name: If the motor carrier service in general has a name different from your business name, write it here. Otherwise, leave this item blank.
3-7. Principal Place of Business: Input here the address where your company is engaged in business operations and where your safety records are regularly maintained. The address you put here will be used by the FMCSA for on-site visits, so that they could conduct monitoring of your company’s adherence to safety standards, such as safety audits and inspections.
8-12. Mailing address: If different from the principal address, please write down the mailing address on the space provided. If the mailing address and principal address are the same, check “Same as Principal Address”.
13-15. Contact numbers: Write here your company’s principal phone number, cell phone number, and fax number. These numbers may be used bythe FMCSA to contact you.
16-19. Identification numbers: Write here the identification numbers of your company (USDOT Number, MC or MX Number, Dun & Bradstreet Number, IRS/Tax ID Number).
20. Email address: Write here your company’s email address for the official point of contact.
21. Carrier mileage: Enter the total mileage of all Commercial Motor Vehicles in your company’s operation for the past 12 months.
22. Company operations: Indicate here your company’s operations type. This would inform the FMCSA the type of service your company will be providing. Select all the types that apply.
23. Operation Classifications: Indicate here the type of business the company is engaged in. This will help the FMCSA determine the regulations that your company is subject to.
24. Cargo classifications: Indicate here the types of materials that your company transports or ships.
25. Hazardous materials: Complete this item only if your company transports hazardous materials. Indicate here the types of hazardous materials your carriers are transporting. If your company does not transport hazardous materials, please skip this item.
26. Number of vehicles: Indicate here the number of vehicles your company uses in its operations. Fill the table with the information needed.
27. Driver information: Enter here the number of interstate and intrastate drivers who operate CMVs for the company on an average workday. Include here your part-time, trip-leased, casual, and company drivers.
28. Is your USDOT number registration currently revoked by the FMSCA: Indicate here your company’s assigned USDOT number.
29. Compliance certification: All Passenger Carrier applicants must complete this section.
30. Name of sole proprietor, partners, or officers and titles: Indicate here the name and title of the company owner/s or corporate officers.
31. Certification statement: Print or type the name of the individual authorized to sign the documents on behalf of the entity listed in item 1.
