DA Form 348 is a determining factor of a service member’s equipment operating and driving qualifications. It is used in the United States to ensure that every service member of the Army is qualified to operate specific types of equipment.
Training results are recorded on DA Form 348. If the results do not fit on DA Form 348, use DA Form 348-1.

SECTION III - PERFORMANCE RECORD
List chronologically; enter first the earliest event.
Date
Enter the date you completed formal training on the equipment you are authorized to operate. In cases where road tests, age, or local requirements are necessary, training may be completed before you qualify in Section I of the DA Form 348. Dates of accidents, awards, and traffic violations are also to be noted here.
Credits
Enter credits such as awards, training, retraining, testing, retesting, roadeos, permit renewal, licensing, etc.
Debits
Enter debit such as accidents, arrests, violations, warnings, revocations, suspensions, etc.

Enter a word or phrase that best describes the type or nature.

Enter any actions taken by the commander to guarantee that each individual maintains a high standard as a vehicle or equipment operator. A commander or supervisor refers to this section for information about your driving or operating performance.

PHYSICAL EVALUATION MEASURES
Enter the visual acuity of your left and right eyes.
Enter a check mark if your visual acuity is qualified. If substandard, enter a cross mark.

Enter “NA” (not applicable) unless the individual has a medical profile.

Note: You will not be disqualified for color blindness. However, if color blindness appears possible, you will be given training on traffic light sequence, observation of other traffic, and other ways to compensate.

A. ROAD TEST - PREREQUISITE
