What is the purpose of DA Form 348?

DA Form 348 is a determining factor of a service member’s equipment operating and driving qualifications. It is used in the United States to ensure that every service member of the Army is qualified to operate specific types of equipment.

Training results are recorded on DA Form 348. If the results do not fit on DA Form 348, use DA Form 348-1.

SECTION I - OFFICIAL QUALIFICATIONS

Type of Equipment

Enter description or model of equipment. If qualified to tow a trailer that is kingpin or pintle connected, indicate “w/trailer”.

Enter in tonnage or passenger capacity.

Enter the date you have been qualified to operate the equipment listed.

Enter activity unit identification code, post, camp, or station where qualification to operate equipment took place.

Name of Examiner

Enter the name of the examiner or qualifying official. The examiner will sign in black ink upon verification of your ability to operate the equipment.

SECTION II - BACKGROUND AND EXPERIENCE

Type of Equipment

Enter the vehicle type you are qualified to operate.

Size

Enter the size of the vehicle.

Type of Driving or Operation

Enter the type of driving or operation; it is highway, city, rural, etc.

Additional Driver’s Licenses

Enter the State or agency that issued the license.

Number of Other Driver’s Licenses

Enter the license serial number and expiration date.

Satisfactory Experience Verified By

Enter the name of the individual who verified your experience.

SECTION III - PERFORMANCE RECORD

List chronologically; enter first the earliest event.

Date

Enter the date you completed formal training on the equipment you are authorized to operate. In cases where road tests, age, or local requirements are necessary, training may be completed before you qualify in Section I of the DA Form 348. Dates of accidents, awards, and traffic violations are also to be noted here.

Credits

Enter credits such as awards, training, retraining, testing, retesting, roadeos, permit renewal, licensing, etc.

Debits

Enter debit such as accidents, arrests, violations, warnings, revocations, suspensions, etc.

Type or Nature

Enter a word or phrase that best describes the type or nature.

Action Taken

Enter any actions taken by the commander to guarantee that each individual maintains a high standard as a vehicle or equipment operator. A commander or supervisor refers to this section for information about your driving or operating performance.

REMINDER

• Cite the DA or local regulation that directed that training be conducted or the regulation under which a safety award was issued.
• Note actions taken for accidents or moving traffic violations regardless of whether they occurred while driving a military or privately owned vehicle to include annotation of license suspension or revocation.
• Do not enter accidents you are not at fault with.
• Enter the printed name of the person making the entry and have him or her initial over his or her name.

PHYSICAL EVALUATION MEASURES

1. Visual Acuity

Enter the visual acuity of your left and right eyes.

Enter a check mark if your visual acuity is qualified. If substandard, enter a cross mark.

2. Field of Vision

Enter the standards obtained in the left and right eyes

Enter a check mark if your field of vision is qualified. If substandard, enter a cross mark.

3. Hearing

Enter “NA” (not applicable) unless the individual has a medical profile.

Enter a check mark if your hearing is qualified. If substandard, enter a cross mark.

4. Reaction Time

Enter the time in the applicable blocks. A reaction time from 0 to 0.60 seconds is acceptable.

Enter a check mark if your reaction time is qualified. If substandard, enter a cross mark.

5. Depth Perception

Enter “NORMAL” if your depth perception is normal. If some extremely abnormal condition is obvious, enter “ABNORMAL”.

Enter a check mark if your depth perception is qualified. If substandard, enter a cross mark.

6. Color Perception

Note: You will not be disqualified for color blindness. However, if color blindness appears possible, you will be given training on traffic light sequence, observation of other traffic, and other ways to compensate.

Enter “NORMAL” if your depth perception is normal. If some extremely abnormal condition is obvious, enter “ABNORMAL”.

DRIVING PERFORMANCE TEST

A. ROAD TEST - PREREQUISITE

For items 1 to 7: Enter a checkmark in the applicable blocks to indicate successful completion of each task. Enter a cross mark in the applicable block when the individual has failed and corrective training is needed. Enter “NA” (not applicable) when any portion of the test does not apply to the vehicle or equipment on which the individual is not being tested.

B. ROAD TEST - SCORED PHASE

Number of Tally Marks on Check list PRT 2678

Enter the number of errors from the checklist recorded during the road test. Errors indicate requirements for additional training.

Road Test Score

Enter your road test score. Subtract the number of errors in the Number of Tally Marks on Check list PRT 2678 from 100 to get the road test score. This score is shown as a percent. The passing score is 75 percent and above.
