Form DA 2028 is a document used by the US Army to request changes in army publications.
Form DA 2028, Recommended Changes to Publications and Blank Forms, is a document used by the US Army to request a change in publications and sponsored forms by the Army Publishing Directorate (APD). The proponent of this form is the Office of the Administrative Assistant to the Secretary of the Army.
Using the Army DA Form 2028 falls under AR 25-30, Army Publishing Program and anything that is released following AR 25-30 can be changed using the DA 2028 Form including AR 25-30.
Army publications, otherwise known as DA publications, are documents published under two different agencies or commands that are authenticated by the Department of the Army. These publications will remain in effect indefinitely unless it is superseded, canceled, or rescinded by the US Secretary of the Army. Digital copies of these publications are kept on the official website of the APD, for anyone who wants to use or reference these publications.
It should be kept in mind that Form DA 2028 requires the proponent that published the publication you are requesting to change. Generally, publications will have that information on the form ready for you to refer to.

Submitting the DA Form 2028 is different depending on what you are filling out the DA 2028 Form for. If you are requesting a change on a publication, submit the form to the Publication Control Officer or Forms Control Officer of the proponent organization. If you are leaving comments and suggestions to help improve certain publications, then refer to the publication itself and look for the paragraph or clause on suggestions. These clauses often have information on how they want to be contacted, and how they want the DA 2028 Form sent.

Comments
You can have multiple comments about the publication you are filling out form DA 2028 for. However, you must include all that is applicable from the following:
Each comment must be distinguishable from one another. Each specific change must be labeled in a different comment.
If the publication has a work package number or data module code (more commonly known as a QR code) make sure to add it when commenting about the publication.
