Section 4 - Email Address
Select “Delete Email Address” if you want all future registration renewals printed and mailed to you. If you want all future registration renewals electronically sent to you, select “New Email Address” and enter your email address on the space provided.
Section 5 - Organ and Tissue Donor
Check the first box if you want to consent to organ and tissue donation and wish to be in the donor registry. Select the second box if you don’t want to consent to organ and tissue donation and no longer wish to be in the donor registry.
Section 6 - Voter Registration Application
To register to vote you must be:

Select “Yes” if you are 18 years of age on or before election day. If not, select “No”.
If you’re moving to Connecticut from another state, you will need to get a new driver’s license. To get a Connecticut driver’s license, you’ll have to go to a Connecticut Department of Motor Vehicles office in person. Connecticut driver’s licenses are only available to those who do not have any outstanding suspensions, revocations, or cancellations on their out of state driver’s license and who have not been previously deemed unfit or unsafe to drive by the CT DMV or an outside court.

If your name on your proof of identity does not match the name on the documents showing proof of identity, then you have to bring along proof of name change. There is no need to take a driving test as long as your current driver’s license has not been expired for more than two years, but you will be required to take a vision test.
If you have a vehicle, you have to update your vehicle’s registration. Connecticut requires that you register your vehicle in the state within 60 days of establishing residency.
If your vehicle’s model year 2016 or older, you will be required to get an emissions test. If not, you will be required to get a VIN verification.
After getting an emissions test or VIN verification, you must visit a Connecticut Department of Motor Vehicles office. Make sure to bring along the following documents:
If you’re moving within Connecticut, you have to update your driver’s license within 48 hours with your new address when moving within the state. You have two options for updating your address on your Connecticut driver’s license: in person or by mail.
If you will file in person, you have to go to a Connecticut Department of Motor Vehicles hub and fill out a Connecticut DMV change of address form in person.

If you have a vehicle, you also need to update your vehicle registration by providing the same change of address paperwork.
Just like with your driver’s license, you can update your vehicle’s registration in person or mail your information to the Department of Motor Vehicles, 60 State Street, Wethersfield, CT 06161-5070, Attn: Data Entry Change of Address Unit.
If you’re moving from Connecticut to another state, you must notify the state of Connecticut. Your Connecticut driver’s license will not be automatically canceled when you move. You may cancel your CT vehicle registration online or by mail.
If you will cancel by mail, send a completed Marker Plate Notice Form (Form E-159) to DMV, Registry Record Section, 60 State Street, Wethersfield, CT 06161-5057.
You have to return your Connecticut license plates and mail them with a written cancellation request to DMV, Attn: Plate Return, 60 State Street, Wethersfield, CT 06161-5057.
