What is Form PPTC-054?

Form PPTC-054 is also known as the Canadian Adult Simplified Passport Renewal Application Form. It is used by Canadians to renew their passport.

You may only use this form to renew a passport only if all of the following conditions apply:

• You were at least 16 years of age at the time of your previous application,
• The information (name, date of birth, sex, and place of birth) on this application form is exactly as it appears on page 2 of the submitted passport, and
• The Canadian passport being renewed:
• Is valid or expired for no more than one year,
• Was or is valid for five years or ten years,
• Is not damaged, and
• Was never reported lost or stolen.

How to Fill Out Form PPTC-054?

Section 1. Personal Information

Line 1: Surname
Provide your surname.

Line 2: Given Name(s)
Provide your given name(s).

Line 3: Surname at Birth
Provide your surname that was given at your birth.

Line 4: Date of Travel
If the date of your travel is known, provide the month and day of your departure. Otherwise, check “Unknown.”

Line 5: Former Surname:
Provide your former surname.

Line 6: Mother’s Maiden Name
Provide your mother’s maiden name.

Line 7: Place of Birth
Provide the city, country, and province, territory, or state (if applicable) where you were born.

Line 8: Date of Birth
Provide the date you were born beginning with the year, followed by the month then the day of birth.

Line 9: Sex
If you’re female, check “Female.” If you’re male, check “Male.”

Line 10: Natural Eye Colour
Provide your natural eye colour.

Line 11: Height
Provide your height in either centimeters or inches and indicate the system of measurement used accordingly.

Line 12: Current Home Address
Provide the number; street; apartment; city; province, territory, or state; and postal or ZIP code of your current residential address.

Line 13: Mailing Address
Provide the number; street; apartment; city; province, territory, or state; and postal or ZIP code of your current mailing address.

Line 14: Telephone (Daytime)
Provide the telephone number that you use during the daytime.

Line 15: Telephone (Other)
Provide other telephone numbers that you use if applicable.

Line 16: Email Address
Provide your email address.

Line 17: Declaration
Provide your signature within the white space provided. Also, provide the date you signed this form beginning with the year then the month then the day of signing. Also, provide the location where you signed this form. Include the city and the province, territory, or state where you signed this form.

Section 2. Previous Canadian Passport
Enclose any Canadian passport issued to you within the last six years.

Line 18: Passport Number
Provide the passport number of the enclosed passport.

Line 19: Date of Issue
Provide the date of issue of the enclosed passport beginning with the year, followed by the month then the day of birth.

Line 20: Date of Expiration
Provide the date of expiration of the enclosed passport beginning with the year, followed by the month then the day of birth.

Line 21: Returning Your Passport
If you would like the enclosed passport to be returned to you, check “Yes, please return it to me.” Otherwise, the enclosed passport will be securely destroyed by the Passport Program to protect your personal information.

Section 3. Canadian Citizenship
Complete this section if you were born outside of Canada between February 15, 1977, and April 16, 1981, inclusive.

Line 21a:
If you are a naturalized citizen of Canada, check “Yes” and proceed to section 4. Otherwise, check “No” and proceed to the next line.

Line 21b:
If one of your parents is born in Canada, check “Yes” and proceed to section 4. Otherwise, check “No” and proceed to the next line.

Line 21c:
If your current certificate of Canadian citizenship was issued after January 1, 2007, check “After January 1, 2007” and submit the certificate with your application.

If your current certificate of Canadian citizenship was issued before January 1, 2007, check “Before January 1, 2007” and complete and submit Form PPTC-001, Proof of Canadian Citizenship—Additional Information, available here.

Section 4. Period of Validity

Line 22: Period of Validity
Choose either a 5-year period of validity or a 10-year period of validity and check the circle of your choice accordingly.

Section 5. Additional Personal Information

Line 23a:
If your address in the last two years is the same as your current home address, check “Same as current home address” and proceed to line 24a.

If your address(es) in the last two years is different from your current address, check “Different from current home address” and proceed to line 23b.

Line 23b:
Provide the address(es) and length of time of residence. Include the number; street; apartment; city; province, territory, or state; country; and postal or zip code of your previous address(es). For the length of time of your residency, include the year and time of your residence.

Line 24a:
Check all the boxes that apply to you for the last two years:
I was employed (full- or part-time)
I was in school (full- or part-time)
Other, e.g. homemaker, unemployed or retired

Line 24b:
Provide the full details of your answer in line 24a. Include the employer, school, or other; their address; their daytime telephone number; your field of employment or studies; and the length of time of employment, schooling, or other.

Section 6. References

Line 25:
Provide at least two people and their contact information who are at least 18 years old, have known you for at least 2 years, and are not your relatives. Note that they must consent to have their contact information provided as they may be contacted to confirm your identity.

Provide their surnames, given names, your relationship with them, address (including the number; street; apartment; city; province, territory, or state; country; and postal or zip code), daytime telephone number, other telephone numbers, email address, the number of years they have known you.

Section 7. Emergency Contact Information (Optional)
It is recommended that you provide the name of someone who would not normally travel with you as this information will be helpful in cases of emergencies.

Line 26:
Provide their surnames.

Line 27:
Provide their given names.

Line 28:
Provide your relationship with them.

Line 29:
Provide their daytime telephone number.

Line 30:
Provide their other telephone numbers

Line 31:
Provide their email address.

Line 32:
Provide their address. Include the number; street; apartment; city; province, territory, or state; country; and postal or zip code of the address.

Line 33: Signature
Provide your signature within the white space provided. Also, provide the date you signed this form beginning with the year then the month then the day of signing.

Submission

Submit your accomplished Form PPTC-054 with two identical and unaltered passport photos taken within the last six months, any Canadian passport issued to you within the last six years, the fee required, and if you were born outside of Canada between February 15, 1977, and April 16, 1981, inclusive, you may be requested to submit further documentation.

Submit the requirements at any Passport Program regional office, any participating Canada Post office, or any participating Service Canada Centre.
