What is a Canada Passport Renewal Form?
A Canada Passport Renewal Form is a document used in order to renew a resident of Canada’s passport for international and domestic travel by plane. The form can also be used to request a new passport, in the event that an old one was damaged or lost, while also cutting down on the amount of time that would normally be spent to apply for and obtain a new passport.

Certificate of Canadian Citizenship Issue
Check the box that indicates when your certificate of Canadian citizenship was issued. If it was issued after January 1, 2007, submit the original certificate alongside this document. If it was issued before January 1, 2007, complete and submit form PPTC 001 alongside this form.
Choose the period of validity between 5 years or 10 years that applies.

Addresses in the Past Two Years
Mark the box that indicates whether your address in the past two years has been the same as your current home address, or if they are different from your current home address. If the latter, enter the following information for each of the addresses you lived in for the past two years other than your current one.

No. Once the passport renewal form is completed and submitted, no changes can be made to it.
What do I do if I realize that there is an error on the form only after I’ve submitted it?
You will need to contact the passport services office that you submitted the form to, and inform them of the error and your intention to correct it. You will most likely need to fill out a new form and pay any applicable fees again.
Who is eligible to use the Canada Passport Renewal Form?
Canadian citizens who are 16 years of age or older and currently possess a Canadian passport that is still valid or has expired for no more than one year are eligible to use the Simplified Renewal Passport Application.

No, if your passport has been expired for more than one year, you cannot use the Simplified Renewal Passport Application. You will need to apply for a new passport using the Adult General Passport Application form.

You can pay the passport renewal fee by credit card if applying online or by debit card, credit card, or certified cheque if applying by mail or in person.
