Form MVT-12-1 or the Alabama Duplicate Title Application form is an application form used by Alabama residents to replace a stolen, lost, mutilated, or illegible vehicle title. This form is also used to add or remove a lien and change the ownership of a vehicle.
Vehicles that are titled in the state of Alabama can apply for a title replacement. You can visit the official website of the motor vehicle division for more information.
VEHICLE IDENTIFICATION NUMBER
Enter your vehicle’s identification number (VIN). It is the identifying code of your vehicle.
YEAR MODEL
Enter your vehicle’s year model.

Enter the brand of your vehicle.

Indicate the number of your vehicle’s cylinder. It is the power unit of an engine.

Enter the number of a new cylinder in your vehicle.

Enter the number of existing liens registered on your current title.
COLOR
Indicate the color of your vehicle. If the vehicle has two colors or multiple colors, enter the predominant color as the major color and the secondary color as the minor color.
ODOMETER READING
Enter the mileage of your vehicle or the total distance traveled.

The owner’s name must be identical as shown on the original title.

In this section, you are allowing to have the certificate of title mailed to the preferred address. if the replacement title will be mailed to someone other than the owner, the owner or his/her authorized representative must complete Section I in its entirety.

CERTIFICATION STATEMENT
Ensure that all the information provided in this application for a replacement is true and correct. False statements are punishable under the Alabama Uniform Certificate of Title and Antitheft Law (Title Law) (Sec. 32-8).
By submitting this form, you acknowledge that the existing certificate title will be voided. If the lost one has been found, you should return it to the Alabama Department of Revenue immediately.
The lienholder must complete this section for requesting the replacement title.
NAME OF LIENHOLDER
Enter the name of the lienholder.
SIGNATURE OF AUTHORIZED REPRESENTATIVE
The authorized representative must sign this application.
By signing this section, you are about to release the lien recorded on the original certificate of title. Then, enter the date.
SIGNATURE OF OWNER AND DATE
The owner must sign and date.
SIGNATURE OF CO-OWNER AND DATE
The co-owner must sign and date.
Note: The owner(s) of the vehicle or owner’s authorized representative must complete Section I unless the replacement title is being sent to the recorded lienholder.

You also need to submit an application fee of $15 in certified funds payable to Alabama Department of Revenue.
