Form 478, Application for Alaska Driver License, Permit, or Identification Card, or referred to as the Alaska Driver’s License Application form, is a legal form in Alaska used to request for a driver’s license, instruction permit, or state identification card from the Alaska Division of Motor Vehicles (DMV). This may serve as proof of identity or another lawful status.
Residents of Alaska who have held provisional driver’s license, without any traffic convictions, for at least six months, can use Alaska Driver’s License Application form.

Non-commercial driver’s license is for a Gross Vehicle Weight Rating (GVWR) of 26,000 pounds or less.

CDL — B is for a commercial driver’s license to operate any vehicle with a GVWR of 26,001 pounds or more with a trailer with a GVWR of 10,000 or less.
