What is the Wyoming Change of Address Form?

Form FSDL-763, Notice of Change Address, or also referred to as the Wyoming Change of Address Form, is used to update an applicant’s physical address on his or her Wyoming driver’s license or identification card. The Wyoming Department of Transportation (WY DOT) is responsible for updating an applicant’s address in their driver record files. An applicant may be able to change his or her address by going to the nearest Wyoming Department of Transportation (WY DOT) office, by mailing his or her application, or by fax.

Within 10 days of applying for a Wyoming Change of Address Form, an applicant will be required to apply for a duplicate Wyoming driver’s license. If the applicant decides to apply for a replacement Wyoming driver’s license or identification card, he or she must apply for it in person by going to a Wyoming Department of Transportation (WY DOT) office.

If the applicant wishes to change the address on his or her motor vehicle records and other documents, he or she must contact the nearest Wyoming Department of Transportation (WY DOT) office since the required procedure may differ depending on which county the applicant resides in.

If the applicant wishes to have his or her Wyoming Department of Transportation (WY DOT) documents updated with the new address, he or she must pay a processing fee to the office.

The applicant must pay the following fees for the Wyoming Department of Transportation (WY DOT) documents stated below:

• Wyoming Driver’s License - A change of address in an applicant’s Wyoming driver’s license costs $35.00.
• Wyoming Commercial Driver’s License - A change of address in an applicant’s Wyoming commercial driver’s license costs $40.00.
• Wyoming Identification Card - A change of address in an applicant’s Wyoming identification card costs $10.00.

If the applicant has recently established his or her residency in Wyoming from a different state, he or she is required to transfer his or her out-of-state driver’s license to the Wyoming Department of Transportation (WY DOT) in one year. On the other hand, if the applicant has a driver’s license from the following states or if he or she has a commercial driver’s license, he or she will have to obtain a Wyoming driver’s license or a Wyoming commercial driver’s license once he or she has become a resident of Wyoming:

• Georgia
• Massachusetts
• Michigan
• Tennessee
• Wisconsin

An applicant will always be required to get a new Wyoming driver’s license unless he or she is a student who is not permanently moving to the state of Wyoming, if he or she is an active member of the United States military, or if the applicant is a dependent of either.

For a formerly out-of-state resident to obtain a Wyoming driver’s license or Wyoming commercial driver’s license, he or she will have to go to the nearest Wyoming Driver Exam Office in person and submit the following documents:

• The applicant’s out of state driver’s license
• The applicant’s proof of identity documentation
  • A valid birth certificate
  • A valid United States passport
  • A valid certificate of citizenship
• The applicant’s proof of current residence documentation
  • Any mail that has been addressed to the client at his or her new address that must be 30 to 45 days old such as electricity bills, water bills, or phone bills
• The applicant’s proof of social security
  • Social security card with social security number
  • Form W-2, Wage and Tax Statement

If the applicant has recently changed his or her full legal name and the said name does not match the name on his or her primary proof of identity, he or she must include the following documents during the submission:

• The applicant’s proof of legal name change
  • A valid marriage license
  • A divorce decree
  • A court order for a legal name change

The required documents to be submitted must be original. The Wyoming Department of Transportation (WY DOT) will not be accepting photocopies of the documents stated above.

Once the applicant has fully established his or her residency in the state of Wyoming, he or she will also be required to have the motor vehicles brought from his or her previous state registered with the Wyoming Department of Transportation (WY DOT).

An applicant will not be required to take a driving test if he or she wishes to have the address on his or her Wyoming driver’s license or identification card changed. However, he or she will be required to take a vision test.

To know if an applicant is officially a resident of the state of Wyoming, he or she must be planning to do the following:

• Live in the state of Wyoming for at least 4 months out of the whole year
• Register to vote in the State of Wyoming
• Work in the state of Wyoming
• Send his or her children to school in the state of Wyoming
