Form L1-1, Application for License, Identification Card and Permit, or the Rhode Island State ID Application Form is a form used to apply for a State ID with the Rhode Island Division of Motor Vehicles (DMV). It can also be used to apply for a driver’s license. The form must be submitted in person at a DMV office, alongside the required documents and payment for the application fee.
State IDs are used in many of the same cases as a driver’s license, such as use as a proof of identity, or as a valid ID for the purpose of purchasing certain products like alcohol and cigarettes. Unlike a driver’s license, however, a state ID does not give the holder the privilege to drive, nor does it contain information on the requirements that the holder must follow in order to drive (i.e. must wear glasses, the type of vehicle(s) that may be operated).

Residents of Rhode Island, whether new or not, must apply for either a state ID or a driver’s license, as many different services, businesses, and government agencies require either one to serve, at minimum, as proof of identity. While the Rhode Island State ID Application Form is very short and simple, it does ask for sensitive information such as your address and social security number. Make sure to keep the form in a safe place to ensure your personal information’s security.
