What is the Rhode Island State ID Application Form?

Form L1-1, Application for License, Identification Card and Permit, or the Rhode Island State ID Application Form is a form used to apply for a State ID with the Rhode Island Division of Motor Vehicles (DMV). It can also be used to apply for a driver’s license. The form must be submitted in person at a DMV office, alongside the required documents and payment for the application fee.

State IDs are used in many of the same cases as a driver’s license, such as use as a proof of identity, or as a valid ID for the purpose of purchasing certain products like alcohol and cigarettes. Unlike a driver’s license, however, a state ID does not give the holder the privilege to drive, nor does it contain information on the requirements that the holder must follow in order to drive (i.e. must wear glasses, the type of vehicle(s) that may be operated).

To be able to fill out a valid L1-1 form, the following requirements must be met:

  • Must be a Rhode Island resident
  • Must not currently have a valid license in Rhode Island or any other state

In addition, submission of the application form must also be accompanied by the following documents:

  • One document as Proof of Identity. Changes in name must also be accompanied by proof in the form of documentation issued by a valid government agency. If you are using a passport and you obtained or renewed it within the last six months, bring an additional identity document in case your passport does not verify with the national database.
  • Two proofs of residency documents. Note that P.O. box addresses are not accepted as valid residency documents.
  • One proof of Social Security Number document.

Submission of the application form must then be accompanied by payment for the application fee. Checks are not accepted as payment for state ID application fees.

Who needs to use the Rhode Island State ID Application Form?

Residents of Rhode Island, whether new or not, must apply for either a state ID or a driver’s license, as many different services, businesses, and government agencies require either one to serve, at minimum, as proof of identity. Driver’s licenses are required for anyone who wishes to drive within the state of Rhode Island.

A state ID is particularly useful for people who do not wish to or otherwise cannot drive, as it confers many of the same privileges that driver’s licenses do.
