What is the Rhode Island Driver’s License Application Form?
Form LI-1, Application for License, Identification Card, and Permit, or the Rhode Island Driver’s License Application is used to apply for an original, renewal, or duplicate learner's permit, driver's license, motorcycle license, or identification card with the Rhode Island Division of Motor Vehicles (DMV).

Required Documents to File the Rhode Island Driver’s License Application Form
According to the official website of Rhode Island DMV, you must provide the following documents:
Proof of Identity
You must present one document from the list below. These documents must have your legal name and correct date of birth.
- A valid, unexpired U.S. passport;
- A certified copy of a birth certificate filed with a State Office of Vital Statistics or equivalent agency in your State of birth;
- A Consular Report of Birth Abroad (CRBA) issued by the U.S. Department of State, Form FS-240, DS-1350 or FS-545;
- A valid, unexpired Permanent Resident Card (Form I-551) issued by the United States Department of Homeland Security (DHS) or Immigration and Naturalization Service (INS);
- An unexpired foreign passport with a valid, unexpired U.S. visa affixed accompanied by the approved I-94 Form or the DHS Arrival/Departure Form documenting the applicant’s most recent admittance into the U.S.;
- A Certificate of Naturalization issued by the DHS, also known as Form N-550 or Form N-570, or
- A Certificate of Citizenship, also known as Form N-560 or Form N-561, issued by the DHS.
If you changed your name, the proof of change provided on your primary identity document should be demonstrated with a document issued by an authorized government agency.
If you happen to obtain or renew your passport within the past six months, you must bring another Identity Document, since your passport may not be verified with the national database.
Proof of Residency
You should present two documents from the list below. P.O. box addresses will not be accepted.
- A utility bill (gas, electricity, telephone, cable, oil, water, sewer, satellite, heat, cellphone) with the address of residence clearly stated in your name or in the name of a member of your immediate family dated within the past 60 days;
- A personal check or bank statement with your legal name and correct address of residence dated within the past 60 days;
- A payroll check with your legal name and correct address of residence dated within the past 60 days;
- A lease agreement currently in effect. The lease agreement should contain your legal name and correct address of residence and your landlord’s legal name, address, and telephone number. Handwritten lease agreements will not be accepted;
- A letter issued by Rhode Island state agency or a federal agency with your legal name and correct address of residence approving your receipt of benefits from an entitlement program dated within the past 60 days;
- A homeowner’s or renter’s insurance policy for the applicant’s home or apartment with your legal name and correct address of residence that is currently in effect;
- An automobile insurance policy for your motor vehicle with your legal name and correct address of residence that is currently in effect;
- A property tax bill for your residence from the immediately preceding year;
- A letter from a Rhode Island shelter or halfway house stating that you reside at the said facility. This letter must be on letterhead, must be dated within the past 30 days, and must contain the name and contact information of the shelter or halfway house administrator; or
- A jury duty summons dated within the past 60 days;
- A W-2 Form, also known as the Wage and Tax Statement Form, with your legal name and correct address of residence from the immediately preceding year;
- An installment loan contract (automobile, student loan) with your legal name and correct address of residence that is currently in effect;
- A current Social Security Administration statement with your legal name and correct address of residence;
- A current pension or retirement account statement with your legal name and correct address of residence;
- A Rhode Island-issued firearms permit with your legal name and correct address of residence that is currently in effect; or
- A valid Rhode Island driver’s license or identification card;
If you are a minor, in addition to any of the proof of residency documents mentioned above, the following documents will also be accepted:
- A school-record from the current school year (report card, diploma, transcript, or ID card) stating your correct address of residence (past year if during summer vacation)
- The Rhode Island driver’s license or identification card of the individual signing your application.
Proof of Social Security Number
Provide your Social Security Number.
New Permit
- Driver Education Certificate; if you have a driving certificate that has not been approved by the Community College of Rhode Island (CCRI), you should take the knowledge exam test. If you are under 18 and applying for a learner’s permit, this LI-1 form must be notarized if your parent or guardian has a different last name than you.
Name Change
- Note: you should wait for 24 hours for your name change to register with the Social Security Administration.
- Current Rhode Island license or identification card
- Original government-issued document that indicates your name change (ex. Marriage Certificate)
Address Change
- Current Rhode Island license or identification card
Rhode Island License/ID/Permit Renewal or Duplicate and New ID
- DMV customers only need to complete form LI-1 or form CDL-1, Application for Commercial Driver’s License, and all required supporting documents stated above. All non-U.S. citizens must bring all supporting documents to be reviewed.
Gender Change
- Gender Designation form
- Current Rhode Island license or identification card
Out-of-State Transfer
- Your current license from the other state; if your out-of-state license is not available at the time of transfer, you should submit a driving record or a type of verification from the other state along with an Identity Document.
Motorcycle Permit
- Certificate of completion from the Rhode Island Motorcycle Safety Course
- Current Rhode Island license
Motorcycle License
- Current license and valid motorcycle permit
