This form is use to notify the OR DMV within 30 days of your move if you've changed your address in Oregon.
If you moved to a different address within Oregon, you are mandated to notify the Oregon Department of Motor Vehicles (DMV) about your address change. To do so, you would have to apply for a change of address by filling out the Form 735-6438 or the Oregon Change of Address Notice for DMV Records.
Your driver’s license, permit, or identification card counts as a valid identification. A valid ID is considered a primary identification and is required in applying for government benefits and services. It is also useful in conducting business transactions to avoid fraud.
A valid ID serves as proof of your identity. It should primarily show your legal name, address, and date of birth. It could also have any other additional information about you, depending on the type of ID you are applying for. Therefore, it is important that the information on your ID is updated. Having false or non-updated information on your ID may be considered as a crime and may result in you being fined or imprisoned.
When you update your address to the DMV, the DMV will change your address on all their records: (1) your Driver License, Permit, or ID card record; (2) your vehicle registration records*, and; (3) your voter registration record (this is optional).
*Updating the DMV of your address change would update the address of all vehicles registered in your name.

The submission of an accomplished form only entails an update of your records in the DMV. You will not be issued a replacement ID nor an address sticker reflecting your current residence address.
