A completed Form DMV 06-104 is needed to renew a NE driver's license/ID card
Form DMV 06-104 or the Nebraska Driver’s License Renewal form is used by residents of Nebraska to renew their driver’s license.
Nebraska’s Department of Motor Vehicles (DMV) sends postcards to drivers 30 days before the expiration of their licenses. It is unlawful to drive with an expired license; being caught doing so results in penalties.
Licenses issued to individuals under 21 years old expire on the card holder’s 21st birthday. If you are under 21 years old, you may apply for renewal 60 days before your 21st birthday, but your license will not be valid until 10 days prior to your 21st birthday.
If you are 21 years old or older, your license will expire on your birthday in the fifth year after its issuance. You may renew your license 90 days prior to your birthday.
To start the process of renewing your driver’s license, you must fill out form DMV 06-104.
The two-page Nebraska Driver’s License Renewal form requires your personal information. Follow the guide below to answer the form accurately:
Complete Information
Date of Birth asks your date of birth in the following format: Month, Day, Year.
Social Security Number asks for your unique nine-digit SSN.
Provide your full legal name in the following format: Last Name, First Name, Middle Initial, Suffix.
Provide your full Current Residential Address.
Provide your full Current Mailing Address if different from your Residential Address.
Provide your County Number.
In the Gender box, select “M” if you are a male and “F” if you are a female.
Provide your Height in Feet and Inches.
Provide your Weight.
Provide your natural Eye Color.
Provide your natural Hair Color.
Select your Race. You may select Black, White, American Indian, Asian or Pacific Islander, Hispanic, or Other if your race is not in the options.
For the purposes of complying with Neb. Rev. Stat. 60-484-04, you can attest if you are a citizen of the United States or not but have lawful status and agree to provide valid documentary evidence.
You may or may not answer the motor voter and veteran questions.
1A — Select “Yes” if you wish to register to vote as part of this application process. “No” if otherwise. You only need to register again if you have changed your name, address, or political party.
1A1 — Select your Party Affiliation. You can select Republican, Democratic, Libertarian, or Non-Partisan.
1A2 — Provide the Last Registration Address City, County, or State.
1B — Select “Yes” if you wish to have the word “Veteran” shown on the front of your operator’s license or state identification card. Doing so would show that you served in the armed forces of the United States. “No” if otherwise. To be eligible, you must register with the Nebraska Department of Veterans’ Affairs Registry.
You may also answer questions on organ and tissue donation.
2 — Select “Yes” if you wish to include your name in the Donor Registry of Nebraska and donate your organs and tissues at the time of your death. Select “No” if otherwise.
3 — Select “Yes” if you wish to receive any additional specific information regarding organ and tissue donation. Select “No” if otherwise.
4 — Select “Yes” if you wish to donate $1 to promote the Organ and Tissue Donor Awareness and Education Fund. Select “No” if otherwise.
Everyone except those who are applying for a state ID Card should answer the following questions:
5A — Select “Yes” if within the last three months you have lost voluntary control or consciousness. Then, provide the date. Select “No” if otherwise.
5B — Select “Yes” if within the last three months you have experienced vertigo or multiple episodes of dizziness or fainting. Select “No” if otherwise.
5C — Select “Yes” if within the last three months you have experienced disorientation. Select “No” if otherwise.
5D — Select “Yes” if within the last three months you have experienced seizures. Then, provide the date. Select “No” if otherwise.
5E — Select “Yes” if within the last three months you have experienced impairment of memory or memory loss. Select “No” if otherwise.
6A — Select “Yes” if you experience any condition which affects your ability to operate a motor vehicle due to loss or impairment of foot or leg. Select “No” if otherwise.
6B — Select “Yes” if you experience any condition which affects your ability to operate a motor vehicle due to loss or impairment of upper body strength. Select “No” if otherwise.
6C — Select “Yes” if you experience any condition which affects your ability to operate a motor vehicle due to loss or impairment of range of motion or mobility. Select “No” if otherwise.
6D — Select “Yes” if you experience any condition which affects your ability to operate a motor vehicle due to loss or impairment of hand or arm. Select “No” if otherwise.
6E — Select “Yes” if you experience any condition which affects your ability to operate a motor vehicle due to a neurological or neuromuscular disease. Select “No” if otherwise.
7 — Select “Yes” if since the issuance of your last license or permit your health or medical condition worsened. Select “No” if otherwise.
If you are applying for a school learner’s permit or a school permit, select “Yes” if your home or the school you attend is in a city of 5,000 or less. Select “No” if otherwise.
Affix your signature and provide the date of signing.
Make sure to provide also all the required documents and pay the appropriate fees when you submit your Nebraska Driver’s License Renewal form.

You must have section B of the form completed by an optometrist or an out-of-state driver license examiner.

You will be asked to upload two documents that show your Nebraska address.
Pay $26.50 and a convenience fee using your card.
