What is the Nebraska Change of Address form?

The Nebraska Change of Address form or Form 06-104 is a document produced by the Department of Motor Vehicles (DMV) for when residents in Nebraska need to apply for a state identification card (State ID Card). Moreover, it can be used for the purpose of renewing or replacing one’s learning permit, driver’s license, motorcycle license, or State ID card.

What is the purpose of the Nebraska Change of Address form?

The purpose of the Nebraska Change of Address form is for residents of Nebraska to apply for a renewal or replacement learning permit, driver’s license, motorcycle license, or State ID card if, in case, the following have been stolen, lost, or damaged beyond legibility. Moreover, residents of Nebraska may also use the Nebraska Change of Address form to report any new information they may have and want to update their learning permit, driver’s license, motorcycle license, or State ID card.

Who should file the Nebraska Change of Address form?

Legal residents of Nebraska who want to file for a renewal or a replacement of their learning permit, driver’s license, motorcycle license, or State ID card must file the Nebraska Change of Address form.

• For new residents of Nebraska:

If you have just recently moved from another state to Nebraska, you will have to register your out-of-state vehicle and transfer your out-of-state driver’s license on or before the thirtieth (30th) day after moving to Nebraska. You will need to accomplish this on or before the deadline or you may be penalized. It is recommended that, if possible, you should complete these tasks instructed by the Nebraska DMV before the deadline.

When should you file the Nebraska Change of Address form?

If you have moved to a different county or changed address in Nebraska, you are required to update your address found in your permit, driver’s license, and State ID Card within sixty (60) days after you moved. In order to update your address, you will have to personally go to your DMV office and provide the following documents and information:

• Your current Nebraska permit, driver's license, motorcycle license, or State ID card.
• An accurately filled out Nebraska Change of Address form.
• Two (2) official documents that include your Nebraska address to supply proof of Nebraska residency.
• Your payment for the corresponding permit or card you are trying to renew or replace.

Once you have successfully submitted and presented the following documents and finished the submission process, your updated Nebraska learning permit, driver's/motorcycle license, or State ID card will be mailed to you within 14 business days. While you wait for your new permit or card, you will be given a receipt that will allow you to operate a vehicle for up to thirty (30) days until you receive your permit or card.

If you have changed your name in Nebraska, you are required to update your name found in your permit, driver’s license, and State ID Card within sixty (60) days after you changed your name. In order to update your name, you will have to personally go to your DMV office and provide the necessary documents and information, as well as your payment and duplicate card fees.

If your permit or card does not get mailed to you within twenty (20) business days, it is recommended that you call the Nebraska DMV to ask about the status of your permit or card.

• Driver's license replacement: $15.50.
• State ID card: $15.50. (You may also have to pay an additional $4.50 for the security fee).
• Registration renewal: fee for registration renewal is dependent on which county you are residing in.
• Duplicate certificate of title: $14.

Then, report your SOCIAL SECURITY NUMBER (SSN).

Using the LAST, FIRST, MIDDLE INITIAL, SUFFIX format, state your full name on the boxes provided.

If your CURRENT MAILING ADDRESS is different from your residential address, provide the full address on the space provided. Also include the CITY, STATE, and ZIP CODE.

Next, report on the corresponding boxes your COUNTY NUMBER, GENDER, HEIGHT (in feet and inches), WEIGHT, EYE COLOR, HAIR COLOR, and RACE.

For the purposes of complying with Neb. Rev. Stat. 60-484.04, attest by selecting between ‘Yes’ or ‘No’ if you are:

1. A citizen of the United States.
2. Are not a citizen of the United States, but do have lawful status and agree to provide valid documentary evidence of such as outlined in 60-484.04.

In the next section of the Nebraska Change of Address form, you will see a series of ‘Yes’ or ‘No’ questions. Do not answer anything that is not applicable to you.

For questions 1A through 4, only motor voters and veterans should be answering it, but answering it is only optional.
