What is Form MVR-4?

When you’re buying a car, you would want to make sure that the car you’re buying is functional and still safe to use. Aside from that, you would want to make sure that there would be no liabilities or debts that would come with buying the car.

This also goes for all the customers who wish to purchase vehicles. Because of this, it has become a requirement for sellers to present their vehicle title to give their customers the assurance that they need.

However, there may be instances where your vehicle title is lost, or you may have not received your title after applying for one. Without your vehicle title, you may not be able to sell your car. You may also not be able to make transactions which would require from you your vehicle title.

But do not worry, having your title lost is not the end for you. You can request for a duplicate vehicle title from the Department of Motor Vehicles if your original title has been lost or you have never receieved it after requesting for one.

A duplicate title can be provided to you if your vehicle title has been lost or if you have never received your vehicle title. You can also request for a duplicate title after you have satisfied a lien, and you’d like to remove your lien record from your title.

To obtain a duplicate vehicle title, you must fill out Form MVR-4 or the Application for Duplicate Title Form.

What is a vehicle title?

A vehicle title is a legal document that serves as proof of ownership of the car. It also provides information about the characteristics of the car, the personal information of the owner, and the vehicle identification number (VIN). The vehicle title is an important document and must be kept in a safe place. It is usually requested when buying or selling a car and paying off a car loan.

The vehicle title also shows your lien records. A car lien is an interest in the car that a car owner pays to another person or institution (for example, you may have loaned your car from the bank). Basically, it is a debt that comes with ownership of the car. In a way, the car serves as an assurance that you will be paying for the debt. If you fail to fulfill your lien, the other party could take your car away from you. On the other hand, when the lien has been fulfilled, you may choose to have your lien record removed from your title.

If you are planning to buy a car, you would want to check the vehicle title if it has lien records in it. When you buy the car with a still existing lien record, the vehicle title will be transferred to you, as well as the responsibility of fulfilling that lien.

How to fill out Form MVR-4?

Before filling out the form, remember to read through the form first to know the type of information the DMV would be asking for and to avoid mistakes when filling out the form.

Vehicle Description
Input here the description of your vehicle--this must be the same with the information written in its title.

Lien Record
In this section, indicate details about your lien record shown on your original vehicle title. Write the date, lien holder, and the address on the space provided. You can input here up to three lien records.

Disclosure Section
In this section, you are to allow the DMV to use this information for disclosure to marketing or solicitation purposes.

Check applicable block
In this section, you are asked to specify which type of document you are requesting. You are also going to certify that you have a valid reason for requesting a duplicate of your vehicle title. Provide the information needed in these items.

You are also asked to provide your signature on the space provided to finalize your application. Write the date and address of the signing of the form.

Affidavit of first lien holder
If you have lien records in your vehicle title, the lien holder must sign the form and agree to have the DMV issue you a duplicate title. The lien holder must provide his or her signature and write the date and the address of the signing of the form.

I have now finished filling out Form MVR-4. Is there anything else I need to do?

After filling out Form MVR-4, you must have it notarized before submitting. Note that the last two sections of the form require a notary signature and seal. Remember to have your accomplished Form MVR-4 notarized before submitting it to the DMV.

Do I have to submit anything with Form MVR-4?

You must also submit your lien release if you wish to have your fulfilled lien records removed from your title.

Is there a fee for requesting for a duplicate title?

The duplicate title has a fee of $15, which you have to submit together with the accomplished Form MVR-4. The DMV accepts payments through checks and money orders only. Do not mail in cash.

If you are requesting for a duplicate title because you have not received your original title, the fee can be waived if the original title was released by the DMV within 60 days of requesting a duplicate.

Where do I submit Form MVR-4?

You can mail your accomplished and notarized Form MVR-4 to the North Carolina DMV Office at NCDMV, 3148 Mail Service Center, Raleigh, NC 27699.

You can also submit your documents in person at the North Carolina DMV Office. If you choose to do so, do not forget to bring proof of identity--it is required to be shown to the DMV.

The DMV will release your duplicate title 15 days after they have received your request. If you have not received your duplicate title after 15 days, please contact the DMV for inquiries regarding your application.
