Form N-400, Application for Naturalization, is an application to become a naturalized U.S. citizen.
Form N-400, officially the Application for Naturalization, is a legal document to apply for US citizenship through the naturalization process.
You may use the Form N-400 if you meet the following:
You need both your USCIS A-Number and your permanent resident date to file Form N-400. If you do not have this information, you should schedule an appointment to obtain this information before you file your Form N-400.
Filling out Form N-400 is simple and only demands that you pay close attention to detail. In any case, you should not have trouble using it, as it has clear instructions on how to input all the required information. It is divided into eighteen (18) parts:
Part 1. Information About Your Eligibility
Select only one from the categories which will be the basis upon which you are applying for naturalization.
Part 2. Information About You
Item Number 1
Provide your full legal name as it is on your birth certificate unless it changed due to a legal reason such as a marriage or court order.
Item Number 2
Provide your name exactly as it appears on your Permanent Resident Card even if it is misspelled or has changed through any legal action since you received your card. If you do not have a Permanent Resident Card, type or print “N/A”.
Item Number 3
Provide other names, aliases, or nicknames that you have used.
Item Number 4
Enter the name that you would like to use in the future if it is different from your present legal name.
Item Number 5
Provide your U.S. social security number (SSN) only if you have one. Type or print “N/A” if you do not have one.
Item Number 6
Provide your USCIS online account number.
Item Number 7
Indicate if you are male or female.
Item Number 8
Provide your date of birth.
Item Number 9
Provide the date you became a permanent resident as shown on your Permanent Resident Card.
Item Number 10
Provide the name of the country where you were born.
Item Number 11
Provide the name of the country where you are presently a citizen. If you are a citizen of more than one country, put the name of the foreign country that issued your last passport.
Item Number 12
If you are applying for a waiver of the English and/or civics test (N-648) based on your disability, select “Yes.” If not, select “No.”
Item Number 13a
Select the box which corresponds to your age and the length of time you have been a lawful permanent resident in the U.S.
Part 3. Accommodations for Individuals with Disabilities and/or Impairments

If you are requesting an accommodation to the naturalization process because of disability, select “Yes.” If not, select “No.”

If you are deaf or hard of hearing, select this box and provide the accommodation you need on the space provided.

Part 6. Information About Your Parents
If neither one of your parents is a United States citizen, skip this part and proceed to Part 7.

If your parents married before your 18th birthday, select “Yes.”If not, select “No.”
Item Number 2
If your mother is a U.S. citizen, select “Yes” and complete Items A. - E. Select “No” if your mother is not a U.S. citizen and proceed to Item Number 3 as directed on the form.

If your father is a U.S. citizen, select “Yes” and complete Items A. - E. Select “No” if your father is not a U.S. citizen and proceed to Part 7.
Part 7. Biographic Information
Item Numbers 1 - 2
Select the boxes that best describe your ethnicity and race.
Item Number 3
Select the values that match your height in feet and inches. Do not enter your height in meters or centimeters.
Item Number 4
Provide your weight in pounds.
Item Number 5
Select the box that best describes the color of your eyes.
Item Number 6
Select the box that best describes the color of your hair.
Part 8. Information About Your Employment and Schools You Attended
Provide information on where and when you have worked or attended school during the last 5 years for the complete time period. Include all military, police, and/or intelligence service.
If you worked for yourself, write “self-employed.” If you were unemployed, write “unemployed.”
Part 9. Time Outside the United States
Item Number 1
Provide the total number of days you spent outside the United States during the last 5 years.
Item Number 2
Provide the total number of trips you have taken outside the United States during the last 5 years.
Item Number 3
Provide information for each trip you have taken outside the United States during the last 5 years. Start with the most recent trip and work backward.
Part 10. Information About Your Marital History
Item Number 1
Select your marital status on the date you filed your Form N-400. If you are single and have never been married, go to Part 11.
Item Number 2
If you are married, indicate if your spouse is a current member of the U.S. Armed Forces.
Item Number 3
Provide the number of times you were married. If you were married to the same person more than one time, count each time as a separate marriage.
Item Number 4
Provide the requested information about your current spouse.
Item Number 5
Select the box to indicate whether your current spouse is a U.S. citizen.
Item Number 6
If your current spouse became a U.S. citizen after birth, select the box that indicates when your spouse became a U.S. citizen and provide the date he or she became a U.S. citizen.
Item Number 7
Provide the requested information if your spouse is not a U.S. citizen.
Item Number 8
Provide the number of times your spouse was married. If he was married to the same person more than one time, count each time as a separate marriage.
Item A. Provide your current spouse’s prior spouse’s full legal name.
Item B. Select your current spouse’s prior spouse’s immigration status.
