The duplicate title can also be applied for in person or by mail. If you choose either of these steps, you'll need to fill out the Duplicate Title Application (Form MVT-8) and sign it. As well, the Release of Lien (Form MVT-12) must be completed.
The MVT-8 document consists of four (4) sections that need to be filled out by the requestee (given that there are no existing liens on the vehicle).
The first section that appears on the document asks for the owner’s information and has four (4) lines of questions that need to be answered.
Line 1 asks for the owner’s FULL NAME.
Line 2 asks for the owner’s DATE OF BIRTH.
Line 3 asks for the owner’s TELEPHONE NUMBER.
Line 4 asks for the owner’s CURRENT MAILING ADDRESS.
The second section that appears on the document asks for the vehicle information and has eight (8) lines of questions needed to be answered.
Line 5 asks for the YEAR of the vehicle.
Line 6 asks for the MAKE of the vehicle.
Line 7 asks for the vehicle’s MODEL.
Line 8 asks for the VIN or the VEHICLE IDENTIFICATION NUMBER.
Line 9 asks for the vehicle’s BODY TYPE.
Line 10 asks if the vehicle is NEW, USED, OR REBUILT.
Line 11 asks for the DATE OF PURCHASE of the vehicle.
Line 12 asks for the vehicle’s LINEAGE.
The third section that appears on the document is ONLY to be filled out by the owner/s if there is an existing lien on the vehicle.
Lines 13 and 16 askfor the LIEN HOLDER’S NAMES if the vehicle is financed.
Lines 14 and 17 ask for the DATE OF LIEN.
Lines 15 and 18 ask for the lien holders’ ADDRESS.
Lienholders are lenders that have interest on the property until the owner pays them in full; it should be noted that the LIEN HOLDERS need to sign Line 19.
The fourth section that appears on the document asks for the signatures of all the individuals who signed Lines 1 a & b.
The same individuals will have to sign Line 19 a & b to confirm that the information they have stated in the document is all true and accurate.
The fifth and last section that appears on the document is for authorizing mailing.
Line 21 asks for the DEALER’S NAME and POSITION.
Line 22 asks for the dealer’s ADDRESS and SIGNATURE.
Line 23 asks for their PLATE NUMBER.
It should be noted that this section is not for use of dealers outside Maine or for private sales. This section is also not for transferring ownership.
It must be kept in mind that failure to provide ALL signatures, information, or documents necessary may result in a delay in processing your application. Making a false statement on the MVT-8 form is also a criminal offense.
A duplicate application must be completed, signed by owner/s and lienholder/s (if any), and mailed with the appropriate fee to the Title Unit of Augusta or taken to a branch office location.
You can replace your title in-person at a local Maine Bureau of Motor Vehicles (BMV) office location, or you can process the same paperwork and submit it by mail. A fee of $33 must be paid when you file Form MVT-8.
Typically, it takes 12 days to process your application but the requestee may pay an additional fee of $10 for expedited processing of the duplicate title.

It should be noted that the MVT-8 is not applicable if the vehicle is a model from the year 1994 or older or if a company is listed on the title.
