• E — Enter E if you are an employee, agent, or contractor of a legitimate business who requests a record of someone to verify the accuracy of their submitted information.
• F — Enter F if you will be using the driving record in a civil, criminal, administrative, or arbitral proceeding in a federal, state, local court, agency, or before a self-regulatory body.
• G — Enter G if you will be using the driving record in a research project to produce statistical reports.
• H — Enter H if you are involved in an insurance support organization that will be using the driving request in investigation claims, anti-fraud activities, rating, or underwriting.

How to file the Kansas Driving Record Request form?

If you requested vehicle registration or title records, send your payment and the Kansas Driving Record Request form to the Kansas Department of Revenue Titles and Registration P.O. Box 2505 Topeka, KS 66601-2505. Walk-in service is also recommended.

If you requested apportioned vehicle records, send your payment and the Kansas Driving Record Request form to the Kansas Department of Revenue Motor Carrier Services P.O. Box 12003 Topeka, KS 66601.

If you requested driver license records, send your payment and the Kansas Driving Record Request form to the Kansas Department of Revenue Driver Solutions P.O. Box 2021 Topeka, KS 66601-2021.

Provide a completed Kansas Driving Record Request form for each request. Payment through checks and money orders must be payable to the Kansas Department of Revenue. There is no refund for a request made in error or if you request for records that are not on file.
