Form 4506-T is a new format used to request tax return information made by the Internal Revenue Service of the United States of America. Taxpayers using a tax year beginning in one calendar year and ending in the following year (fiscal tax year) must file Form 4506-T to request a return transcript.
Form 4506-T, Request for Transcript of Tax Return, is a form by the Internal Revenue Service (IRS) that U.S. taxpayers may use to request a tax return transcript from the federal tax agency.
Use Form 4506-T when you want to request one transcript. The IRS only allows one transcript per Form 4506-T. If you are requesting two or more types of transcripts, submit one form per request.

Form 4506-T is a two-page form. The first page requires your taxpayer-related information and asks the type of transcript you want to obtain; the second page contains instructions on how to fill it out.
Answer Form 4506-T accurately and correctly to avoid any problems with your request.

Enter your full legal name, as it appears on your tax return. If a joint return, enter the name shown first.

Enter your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN). If a joint return, enter the first SSN, ITIN, or Employer Identification Number (EIN).
Only enter your EIN if your request relates to a business return.

Enter your spouse’s name as shown on the tax return if a joint tax return.

Enter the second Social Security Number or Individual Taxpayer Identification Number if a joint tax return.

Enter your current address, including apartment, room, or suite number, city, state, and ZIP code.
If you use a P.O. box address, enter it.

Enter the address you used on your last filed return if applicable.

Enter up to 10 number characters to create a unique customer file number that will appear on the transcript.
Answering Item 5 is not required; however, if you answer it, make sure to not use your SSN.

If you need a copy of Form W-2 or Form 1099, you must contact the payer first. If you want to get a copy of Form W-2 or Form 1099 filed with your return, use Form 4506, Request for Copy of Tax Return, and request a copy of your return.

Tax Return Transcript
The Tax Return Transcript is only available for the current tax year and returns processed during the prior three years. It provides the information from your original return documents — Form 1040 series, Form 1065, Form 1120, Form 1120-A, Form 1120-H, Form 1120-L, and Form 1120S — as filed.
Tax Account Transcript
The Tax Account Transcript is available for the current tax year and up to 10 prior years. It provides basic data, such as return type, marital status, adjusted gross income, taxable income, and payment types.
Record of Account Transcript
The Record of Account Transcript combines the tax return and tax account transcripts. It is available for the current year and three prior tax years.
Wage and Income Transcript
The wage and income transcript shows data from information returns, such as Form W-2, Form 1099 series, Form 1098 series, or Form 5498 series. Current tax year information is not available until the year after it is filed with the IRS.
Verification of Non-Filing Letter
The Verification of Non-Filing document is proof that you did not file a return for the year you requested, confirming that the IRS has no record of your Form 1040, Form 1040A, or Form 1040EZ. Take note that current-year requests are only available after June 15th.
You can request to receive your return transcripts electronically or by mail using the Get Transcript feature on the IRS’s official website. Using this option requires that the primary taxpayer on the return is the one making the transcript request.

While waiting for your requested return transcript, take note that the IRS does not call, email, or send text messages. If you receive a random message from anyone claiming that he or she is from the federal agency, chances are it is a scam; thus, you should not respond.
